U.S. and Zambian officials signed a new global health agreement in Lusaka on October 8, marking a significant shift in the United States' approach to international health cooperation. The agreement, known as a Memorandum of Understanding (MOU), was described by Mich Coker, the U.S. Embassy's Chargรฉ d'Affaires, as a transition from a traditional donor-recipient relationship to a partnership based on Zambian leadership and shared accountability.

This change comes as the Trump Administration's second term has seen a chaotic overhaul of U.S. foreign aid, particularly after the dismantling of the U.S. Agency for International Development (USAID) in early 2025. Experts argue that this upheaval has led to severe disruptions in health services in many of the world's poorest countries, including a significant decline in access to HIV medications and the closure of clinics. Janeen Madan Keller, acting co-director of the Global Health Program at the Center for Global Development, noted that the initial months of the second term were marked by confusion and dire health consequences for vulnerable populations.

However, following a period of instability, there has been a movement toward establishing a clearer global health strategy. The State Department has begun to outline the America First Global Health Strategy, which aims to set specific goals and timelines for U.S. health initiatives abroad. Jennifer Kates, senior vice president at KFF, highlighted that this new strategy is emerging amidst ongoing discussions about the role of Congress, particularly a Democratic-controlled body, in shaping this international health policy.

As the Trump Administration's new health strategy unfolds, it remains to be seen how it will be received by Congress and whether it will garner bipartisan support. The implications of these changes are significant, as they could redefine U.S. engagement with global health issues and impact millions of lives in countries that depend on American aid and support.
